Umar Ibn al-Khattab [ra] said: "Hold yourself accountable before you are held accountable and weigh your deeds before they are weighed for you."

This is a sample self-evaluation form you can use to assess your present state in different aspects of life and how you can either change directions, or improve on the good.

SECTION ONE:

Your personal relationship with Allah:

  1. How often did u remember & thank Allah in times of happiness and joy?
  2. Did your understanding of and commitment to Allah deepen with knowledge and practice this year?
  3. Were you lazy in how you related to Allah?
  4. How often did you "talk" to Allah through Dua, apart from when you wanted something?
  5. How often did you ask for Allah's guidance?
  6. How often did you turn to Allah in times of anxiety, need and difficulty?
  7. What can you do on a daily and weekly basis to increase remembrance and commitment to Allah?
  8. How can you memorize relevant Duas from the Quran and Ahadith to remember Allah at all times?
  9. Have you invited other human beings to relate to Allah?

Your Islamic knowledge:

  1. Did you read Quran with meaning and understanding this past year on a daily basis? If not, why?
  2. Did you attend a regular Islamic study circle (Halaqa) or class? If not, where can you find one?
  3. Did you travel to seek knowledge, whether that was to conferences or special seminars?
  4. What are 10 new things that you learned about Islam this past year, which you did not know before?
  5. Did you seek knowledge only for Allah's sake, or was there another reason?
  6. What 10 aspects of Islam would you like to know about by next Ramadan, which you do not know about right now?
  7. How many Islamic books did you read this past year?
  8. How many Islamic videos did you watch?
  9. How many Islamic audiocassettes did you listen to?
  10. What way of learning suits you best (i.e., oral, visual, and written)?
  11. Have you read about people around you?

Your prayers:

  1. Do you pray five times a day?
  2. Did you pray on time at school or at work, or did you make up the prayers when you got home?
  3. Are you praying correctly or just assuming the way you pray is correct?
  4. Have you been concentrating in your Salah?
  5. Have you approached prayer with anticipation, love and fear of Allah, or with a reluctance and dread, hoping to just get it over with?
  6. Do you read Sunnah prayers with the Fard as well?
  7. How can you improve your prayer?
  8. Have you made du�a for your neighbors and colleagues?

SECTION TWO:

Your Family:

  1. When did you last thank Allah for your family?
  2. Is your relationship with your family for the pleasure of Allah?
  3. Have you understood your responsibility as a Muslim to your family?
  4. Do you know your duties as a father/mother, husband/wife, brother/sister, son/daughter?
  5. Do you know your rights as a father/mother, husband/wife brother/sister, son/daughter?
  6. How is your relationship with your parents?
  7. If living together, do you have meaningful conversation with them daily?
  8. Have you developed any grudges that have to be cleared away?
  9. Is your behavior towards them as respectful?
  10. In the last year, what 5 things have u done to bring them closer to Allah?
  11. How have you served your parents?
  12. If they live far away have you called and visited them regularly?
  13. Have you given them gifts on Eid and tried to get together with them?
  14. If they have passed away, when did you pray for them?
  15. Have you done any charity lately for their sake?
  16. How is your relationship with your spouse?
  17. Do you often have meaningful conversation among yourselves daily?
  18. Have you learned about Islam together by attending a regular Islamic activity?
  19. How have you tried to make Islam part of your lives as a couple and family?
  20. Is there a silent grudge that needs to be discussed and cleared away?
  21. How often have you spent leisure time alone together in the past year?
  22. Has there been verbal, emotional or physical abuse in the relationship? If so, why and what can be done about it?
  23. Has your Iman (faith) in Allah and Islam grown through your relationship this past year?
  24. How would you feel if you are in your spouse's place?
  25. How is your relationship with your children?
  26. What is going on in their lives in school, extracurricular activities, social activities, etc.?
  27. Do you know who their friends are?
  28. Do you talk to them on a daily basis in any meaningful way?
  29. Do you spend leisure time with them?
  30. What five things have you done to bring them kids closer to Allah?
  31. Has there been verbal, emotional or physical abuse in the relationship? If so, why and what can be done about it?
  32. Have you been living example of Islam to them?
  33. How can you spend more time with them?
  34. Do you know what kind of environment they are growing up in?
  35. Do you know what their favorite television shows, writers, video games, toys, etc., are?
  36. How many Friday Prayers have they prayed in the Mosque?
  37. What do you think they think about you?
  38. Are they likely to talk to you or they rather talk to your spouse when they need to?
  39. How is your relationship with the relatives (i.e., brothers and sisters, etc.)
  40. How often do you contact them?
  41. Did you send them Eid cards/gifts or at least call on Eid?
  42. Do you visit each other at least once a year (if you're not living close by)?
  43. Are there any grudges that need to be cleared up?
  44. Have you tried to bring them closer to Allah?
  45. Have you helped them with Halal needs (i.e., financial help)?
  46. Do your children know your brothers and sisters?

SECTION THREE:

Your money:

  1. Have you increased your wealth during last year?
  2. How have you thanked Allah for it?
  3. Is your income (i.e., job) Halal? If you're not sure, who can you consult about this?
  4. Did you pay your Zakat?
  5. How often and how much (approximately) did you spend in charity?
  6. What kinds of things, people, causes did you spend on this year?
  7. Do you keep track of expenses?
  8. Do you have a personal budget?
  9. Has your debt increased or decreased?
  10. Have you reduced taking and giving Riba?
  11. What are the top five worthy causes to give to next year?

SECTION FOUR:

Your health:

  1. Have you thanked Allah for your health?
  2. Have you exercised regularly? If not, why not?
  3. Do you eat properly?
  4. Do you have a balanced diet?
  5. Do you regularly pray extra prayers to worship Allah with heart AND body?
  6. Do you know how to take care of your health?
  7. Have you encouraged family and friends to do the same?
  8. When did you had your last general checkup?

SECTION FIVE:

Your Career:

  1. Have you lost focus and direction in your job?
  2. Do you like your job?
  3. Is your job Islamically acceptable?
  4. If not, what kind of career change can you make that will provide you with Halal income while using your skills for the sake of Allah?
  5. Have you consulted a qualified person about this?
  6. If your job is Halal, have you tried to improve productivity and dedication to your career?
  7. What new ways can you use your career to improve your relationship with Allah, benefit the Ummah, and humanity?
  8. Would you recommend my career to a practicing Muslim?

SECTION SIX:

Your education:

  1. What are your current qualifications?
  2. Are you satisfied with them?
  3. If not, what level of education do you want to attain?
  4. What are the Halal means (i.e., interest-free loans) available to further your education?
  5. If a student, is your field of study one that will bring you closer to Allah?
  6. How will your field of study benefit Muslims and humanity in general?
  7. Have you studied with enthusiasm and dedication?
  8. Any additional programs or courses you should take to improve your level of education?

SECTION SEVEN:

Your time:

  1. What did you spent the most time on this year?
  2. Are you satisfied with this?
  3. If not, where do you want most of your time to be spent?
  4. What changes do you need to make to accommodate this?
  5. Do you have good time management skills?
  6. If not, what can you do about it?
  7. Can you read a book about it?
  8. Can you spend some money doing a course on time management?
  9. Do you plan for your time?
  10. What would be an example of a day in your life where time was well spent?
  11. How much free time did you have on average per week this year?
  12. What is the number one activity you participated in? Are you satisfied with this?
  13. How many hours of free time do you currently have?
  14. How can you better spend free time?
  15. How can your free time make you closer to Allah?
  16. How much TV do you watch?
  17. Do you spend more time watching TV or talking to your spouse and children?
  18. Can you reduce your time for TV?

SECTION EIGHT:

Your work for Islam:

  1. How much time do you volunteer for good causes?
  2. Have you volunteered you time for your Mosque or Muslim school?
  3. Have you volunteered for any soup kitchen or thrift store?
  4. How much time have you spend conveying the message of Islam to others?
  5. Have you prayed for those Muslim who do not pray and fast?
  6. Have you visited your friends and relatives who are away from Islam?
  7. Have you or your neighbors participated in each other�s happiness or sorrows?
  8. Have you developed any skills, which a Muslim organization can use?
